Dimples at the side of the mouth can be an endearing feature, providing enhanced facial aesthetics with smiling. Dimples are considered attractive across many cultures. A natural dimple is formed when there is a small gap in the cheek muscle, which allows there to be a small indentation at the corner of the mouth when a person smiles. Dimple creation surgery can recreate this naturally occurring process.

At Ashlin Alexander Facial Cosmetic Surgery, we can create dimples in a short procedure performed under local anesthesia. A small, painless incision is made on the inside of your cheek. The procedure is performed from the inside of your mouth with no incisions or scars on the outside skin. We can do one side or both sides. Downtime can be lengthy, depending on the individual, but the outcome of new dimples is unforgettable!

The Consultation Process

When you come in for dimple surgery, your first step will be a private consultation with Dr. Alexander. He will determine the specific characteristics in a dimple that you find attractive. Because dimple surgery is specifically tailored to your face, a discussion about the exact location of the dimples as well as the shape and depth is very important.

Some patients desire rounder, deeper dimples. Examples of celebrities with dimples like this are Mario Lopez or Miranda Kerr. Others desire a deep but more vertical dimple such as those possessed by Kirsten Dunst or Jennifer Garner. Some prefer a much more subtle dimple that is only seen with a very full smile. All of these variations are possible by varying the depth and direction of the dimple creation surgery.

The Dimple Surgery Procedure

Dimpleplasty is performed under local anesthesia in our procedure room, which has met the highest standards of certification from the Ontario College of Physicians and Surgeons for cosmetic facial plastic surgery procedures.

The first step is to mark the exact location of the desired dimples on the outside of the cheek. A very small amount (approximately 1cc) of local anesthetic is placed on the inside of both cheeks, and then a tiny incision is made on each side. A small amount of muscle (approximately 3 to 4 mm) is removed in the location that it would be absent in a natural dimple. Next, one or two absorbable sutures (which dissolve in 2-4 weeks) are used to create the new dimple. The amount of muscle removed and the depth of the sutures need to be adjusted very precisely to create the most natural appearing dimples. The entire dimple surgery procedure takes 30-45 minutes to complete.

Dimple Surgery Recovery

The recovery process can vary from person to person. In general, it is a smooth process with very little pain and some short-term swelling, similar to undergoing a dental procedure.

For the first week, you will want to wear a small Band-Aid over the dimples as they will be quite deep. Initially, we make the dimples very deep because we know they will relax during the healing process and we want to compensate for this relaxation.

For the first 3-4 weeks, the dimples may be visible at all times but this subsides once the healing is complete. Eventually, they should relax to a point that they won’t be visible at rest. It typically takes 3 months before the dimples are 100% fully healed.

You can resume your normal activities immediately after the procedure, and can eat and drink normally within one or two hours, once the freezing has worn off. We recommend a soft diet for 3 days and regular cleansing with a mouth rinse for the first week.

During the first two to three days after dimple surgery, there can be some swelling in the cheeks and sometimes a little bit of bruising right at the location of the dimple. This is typically fairly minimal, but some patients do elect to take 2-5 days off from work or school. We prefer to perform the dimple creation surgery on a Friday to allow you to take advantage of the weekend for recovery.
